Why we built Carebrain.
Our story
We kept seeing the same thing in post-acute care: the resident's story broke apart at every handoff. Referrals arrived as faxes, nurses wrote notes twice, and a nurse on the overnight shift waited on a callback while the answer sat in someone else's system.
Vendors built the tools around the people doing this work for hospitals, for billing or for one role at a time. So we built Carebrain: one place where nurses, physicians and APPs, and the teams running the building see the same resident at the same moment, and move care forward together.
